Our goal is to help you live independently, as safely as possible, in your home environment.
Our healthcare services are provided under the direction of your physician and include skilled nursing services, rehabilitation medicine (physical, occupational, and speech therapy), medical social services, and home health aides. Services are intermittent in nature and include but are not limited to the following programs:
Nursing services include:
Physical Therapy
Our therapists provide care to patients who have functional problems resulting from illness, injury or surgery that limits their abilities to move and perform functional activities. Therapists will evaluate and treat movement dysfunction and use interventions such as therapeutic exercise, functional training, assistive and adaptive devices. All plans of care are established with each patient’s physician and periodically reviewed and revised based on the patient’s progress.
Occupational Therapy
These therapists work with patients to help improve their basic motor functions and reasoning abilities, and also teach patients/caregivers how to compensate for permanent loss of function. The ultimate goal us to help patients perform activities of daily living safely and independently in their home environment.
Speech Therapy
Speech therapists assess and treat disorders related to speech-language, cognitive-communication, swallowing, and fluency. Speech-language and swallowing difficulties can result from a variety of causes including stroke, brain injury or deterioration. Our therapists develop a specialized plan of treatment based on each patient’s needs, in collaboration with each patient’s physician.
Medical Social Services
Our medical social workers can assist you with social, emotional, and/or financial problems that are impacting on your plan of care and impeding recovery from your illness. Social workers can assist you with counseling, plans for discharge and assist in community resource referrals for ongoing needs.
Home Health Aides
Home health aides provide intermittent personal care services, such as bathing, grooming, and vital signs. All health aide services are supervised by registered nurses.
