
Home and Community Based Services, funded by the Department of Public Welfare, Office of Long Term Living and the Medical Assistance Waiver, is targeted to help eligible adults 18 and older, who are physically disabled, live as independently as possible, remain in their home and seek and/or maintain employment. Waiver Coordination assists with access to Home and Community supports and services.
Many disabled and elderly adults cannot remain in their own homes without some assistance with personal care and light household tasks. These services can help them avoid unnecessary institutionalization when a relative or friend is unavailable or unable.
Allied Services Waiver Coordination is free for financially eligible persons. For those over the eligibility limits, fees are based on a sliding scale. Allied Services Waiver Coordination’s professional staff will be happy to assist individuals in identifying the funding sources which can help them take advantage of the many Home and Community Based Services available.
Allied Services Waiver Coordination’s staff provides assessment and scheduling of services which are tailored to the individual and varied needs of their consumers. Anyone can make a referral to Allied Services Waiver Coordination. For more information, please call 877-646-4663.
Start the referral process by contacting our admissions office. Referrals can be made online, phone, fax, or email. Our admissions team can begin the admission process within 24 hours of receiving a referral. We offer this service free of charge and often serve as the referring facility’s primary educator and resource for family services.
