Allied Services Integrated Health System’s 369-bed Skilled Nursing & Rehab Center provides comprehensive care for both short and long term patients. For many patients, it serves as a transition program, rehabilitating them for their return home with home health or outpatient services, or to an inpatient stay at Allied Services Rehab Hospital or Heinz Rehab Hospital, if appropriate.

Long-Term Care:

Skilled nursing care is designed for people who can no longer live safely and independently at home or at an assisted living facility. For patients requiring an extended placement, our long term care program offers specialized nursing care, diverse recreational activities and supportive social services catered to the patient’s individual needs. The physician oversees the medical care along with the interdisciplinary team of licensed professionals. Through the planning process, we are able to follow a patient-centered approach that coordinates the patient’s goals and lifestyle plan with our treatment.

Short-Term Rehab:

Our short-term rehab track provides patients with intensive, 24-hour nursing and/or therapy services, or as a bridge between an acute care hospital stay and their return home. Often times, patients cannot tolerate or do not require the intensity of therapy delivered in an inpatient rehab hospital. Alternatively, a patient’s insurance benefit may not allow inpatient rehabilitation for their condition. Short-term rehab patients individualized occupational therapy, physical therapy, and/or speech therapy in amounts that fit their endurance level and medical condition. Patients may be discharged home with outpatient rehab or home health services, or transitioned to inpatient rehabilitation with the goal of returning home.

Respiratory Care:

Allied Services Skilled Nursing & Rehab Center is the only skilled nursing center in the region that offers a dedicated respiratory unit. The Respiratory Unit is staffed 24 hours/day, 7 days/week with licensed respiratory therapists, who provide extensive pulmonary care for the most medically complex patients, both on a post-acute and chronic long-term level. The 26-bed state-of-the-art unit is staffed by a highly qualified and specially trained team that includes critical care nurses, licensed respiratory therapists, and rehabilitation professionals.

Short-Term Pulmonary Rehabilitation:

The Pulmonary Rehabilitation Program is a specifically designed resident-focused program that helps the resident manage and minimize the debilitating effects of pulmonary disease. Residents who actively participate in the program will gain a better understanding of the disease process, techniques to better manage stress and anxiety and knowledge of how nutrition and diet relate to their medical conditions. Residents will achieve increased strength and endurance and improve their abilities to perform activities of daily living through methods taught to conserve energy.. Both restrictive and obstructive pulmonary conditions we treat include:

  • Asthma; Bronchiectasis; Chronic Bronchitis
  • COPD; Recent Pneumonia; Pulmonary Hypertension
  • Emphysema; Pulmonary Vascular Disease; Lung Resection
  • Interstitial Fibrosis; Pulmonary Fibrosis; Lung Transplant

Any resident with a pulmonary diagnosis that is medically stable is ambulatory and has the potential to return home is a candidate for the program. Residents must be able to tolerate nasal oxygen and attend therapy sessions based on their level of function. Every resident is issued an individualized home program to follow and post-discharge services are coordinated based on resident need. Residents may receive referrals to any of the following: a Home Health Agency; an Outpatient Clinic; a Wellness Program

Memory Care:

The Skilled Nursing & Rehab Center Memory Care Program specializes in the treatment of individuals with Alzheimer’s disease and other senior dementias. This program stresses the importance of improving and maintaining the quality of life for its residents. The 61-bed Alzheimer’s unit provides long-term, short-term, and respite care. In this unit, patients will have access to a structured dementia program through the recreation therapy department.

Our memory care program maintains a high staff-to-patient ratio. We go to great lengths to ensure every resident is looked after by compassionate and trusted professionals who are committed to helping patients. Family members and loved ones are encouraged to visit during appropriate hours, and our staff is always happy to answer questions about a resident’s wellbeing.

Nursing Services:

Rehabilitation Programs:

As a comprehensive provider of award-winning rehabilitative care, Allied Services facilities have a distinct focus on rehabilitation. Our Skilled Nursing & Rehabilitation Center is no exception. Our experienced, credentialed therapists will provide physical, occupational and speech therapy as appropriate for the following conditions:

  • Amputation
  • Arthritis
  • Balance disorders
  • Cancer
  • Hand and upper extremity conditions
  • Orthopedics
  • Lymphedema
  • Parkinson’s disease
  • Speech & swallowing conditions
  • Stroke
  • Respiratory

Recreational Therapy:

the overall goal of recreational therapy is to promote physical, mental, and spiritual wellness and optimize each person’s quality of life. Upon admission, patients receive an activity assessment to identify their interests for participation in physical activities, spiritual services, and social events. The following are some of the services that the Recreational Therapy Department provides:

  • Community events; Creative arts; Seasonal theme celebrations
  • Entertainment; Pet therapy;
  • Multi-faith spiritual services with a private chapel
  • Internet/computer access on all units
  • Coordination of outside shopping for residents

Social Services:

Our Social Services Department serves as the liaison for each resident with all team members to ensure that his or her pathway goals are on target. There is a social worker assigned to each unit who assists family members. The following are some of the services that the social services department provides to each resident:

  • Serve as a resident advocate.
  • Monitor mood, behavior, and psychosocial well-being
  • Provide services planned to restore residents to their optimal level of social health and adjustment
  • Conduct individual and group conferences for residents and families
  • Facilitate support groups to meet resident and family needs in conjunction with the Recreation Department
  • Coordinate spiritual support referrals
  • Coordinate supportive counseling referrals with the on-site counselor
  • Coordinate each resident’s discharge, using community agencies and resources

Dietary Services:

The Dietary Services Department prides itself on providing nutritionally balanced meals that appeal to the diverse needs of our residents. Upon admission, every resident receives a nutritional assessment by the Dietitian to establish dietary needs and meal preferences.

Our admissions team can begin the admission process within 24 hours of receiving a referral.
