What Is Home Care?

Now, more than ever, the ability to receive care and services from the comfort of your home is a benefit. For seniors and other high-risk individuals, it removes the need to leave home and reduces exposure to harmful viruses.

Home Care services can help to reduce falls, readmissions, hospitalizations, and isolation. Whether recovering from an injury, living with a chronic illness, needing additional support to live safely at home, or seeking end-of-life care that allows you to remain with your family, there is a Home Care service that fits.

Home Health

Home health services are used for people who can live safely at home, but require the services of a skilled clinician. Patients who require home health services are medically homebound and generally only leave their homes for physician appointments. Typically, patients receive a combination of nursing, therapy, medical social work, and or home health aide visits over a 60-day period. After Home Health services many people are discharged with no further services. Others may transition to outpatient rehab, or continue Home Health services for longer periods of time to manage a chronic health condition. Services include:

  • Nursing
  • Therapy – Speech, Occupational and Physical
  • Medical Social Services
  • Home Health Aides

In-Home Services

In-Home Services trained compassionate attendants assist with personal care needs (everyday activities like dressing and grooming), homemaker services (meal preparation, shopping, laundry, and light housekeeping tasks), respite care, fiscal management services, and personal emergency response installation and maintenance. These services allow the elderly or disabled to live independently in their home. Services include:

  • Homemaker Services
  • Personal Assistance Services
  • Respite Care
  • Help with Activities of Daily Living

Hospice Care

Hospice care is for patients who are in the final stages of life. Instead of “curing” the patient’s condition, treatment is focused on improving their comfort and quality of life. We use a holistic approach to hospice care designed to address physical, mental, emotional, and spiritual needs. Counseling and bereavement services are also offered for friends and loved ones. Services include:

  • Routine and continuous care
  • Nursing services
  • Counseling, spiritual and bereavement support

Palliative Care

Palliative care is for patients with serious medical conditions who can benefit from additional care alongside curative treatments. Treatment here is again focused on improving patient comfort and helping patients manage their symptoms. Services include:

  • Assisting in coordinating medical care
  • Guidance with treatment options and medical goals
  • Counseling and support for family and caregiver stress

NEW* Therapy Your Way

Therapy Your Way was developed in response to the Corona Virus pandemic to provide alternative or supplemental ways for patients to access the kinds of services that are available at our Outpatient Rehab Centers. For example, someone in need of physical therapy can now mix and match between therapy at an Outpatient Center, therapy in the home with a visiting therapist, or therapy via telehealth.
