In-Home Hospice Care

Supporting Patients in the Final Stages of Life in the Comfort of Their Home

In-home hospice care is recommended for patients in the Routine Care and Continuous Care stages of hospice care. In hospice care, the care team’s goal is to increase the patient’s comfort and quality of life in whatever ways are possible. For some patients, staying in a medical facility will be the best place to accomplish this. Others, however, can receive better care in their homes or current nursing facility.

Allied Services Integrated Health System’s in-home hospice care provides many of the same services you would find at a nursing or hospice facility.

In-home hospice care may include:

  • Pain management
  • Symptom management
  • Emotional and spiritual counseling
  • Therapy sessions
  • Assistance with daily tasks
  • Nutritional services
  • Shopping and other errands

Services for Family & Caregivers

Looking after someone in the final stages of life is a full-time job that requires a team effort. This team often involves family, loved ones, physicians, registered nurses, social workers, and counselors. It is important that caregivers do not neglect their own health and wellbeing during this stressful time. We offer education and counseling sessions for family and caretakers so they can learn more about what end-of-life care involves. These services also help the care team prepare for life after their loved one has passed. We also provide support so that the patient’s care team can take breaks and look after themselves.

Once a loved one has passed, the bereavement process begins for their family. Our team has helped many families cope with the loss of a loved one.
