Intermediate Care Facilities for Individuals with Intellectual Disabilities

Allied Services offers one of the most comprehensive residential systems in Pennsylvania. Through its Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/ID), Allied Services addresses the unique needs of individuals with developmental disabilities and assists them in reaching their fullest potential.

The Developmental Services Division clinical and management staff bring a wealth of knowledge and experience to every program. Each Interdisciplinary Team is comprised of members with over 20 years’ experience at Allied Services in the field of developmental disabilities. Our administration team represents more than one hundred years of designing, developing and managing programs for individuals with significant mental and physical disabilities.

Interdisciplinary Team

Intermediate care offers an alternative to institutional living and provides the assistance needed when family care becomes too difficult. All individuals receive the benefit of the Interdisciplinary Team process headed by a Qualified Intellectual Disability Professional (QIDP) who is responsible to integrate, coordinate and monitor each client’s active treatment program. Along with the individual, members of the team include medical, dietary, psychology, day rehabilitation, recreation, residential, social services, and family.

All individuals receive continuous medical care provided by a Medical Director, Licensed Practical Nurses, and Registered Nurses. Individuals receive prompt treatment for acute and chronic health conditions and services indicated by his/her health status. Medical services are provided as necessary to maintain an optimal level of health for each individual.

Staff Development

All direct care staff receives classroom and on-the-job training to best equip them to work with our individuals. Training includes, but is not limited to, Client Rights and Liberties and Management of Abuse, the IDT process, health services including signs and symptoms of illness, dietary requirements and special concerns, Positive Approaches and Everyday Lives, communication, behavioral support plans, fire safety, and CPR.

Positive Approach

All Intermediate Care Facilities for individuals with developmental disabilities provide continuous qualified supervision and medical care, ongoing psychological and social services and active treatment. All residential programs embrace the principles of Positive Approaches.

Goals of residential programs include:

Empowering patients to make their own decisions
Helping patients achieve personal goals
Helping patients develop relationships
Helping patients feel integrated and active in their community

Allied Services Facilities provides residential and day programming services to individuals age 18 and over who meet applicable admission criteria. Referrals are reviewed by an admissions committee and interviews are conducted on an ongoing basis.

Residential Services

Allied Services provides its ICF/ID services at several locations throughout Lackawanna County. IDPs supervise an interdisciplinary team process where each individual receives active treatment based on assessments and training in all activities of daily living (bathing, dressing, transferring, grooming, maintaining a home, etc.).

William Warren Scranton Residence which opened in 1970, is located on the Morgan Highway Campus in Scranton and is home to 42 individuals. This residence includes an 8-bed annex which provides additional living space in an apartment-like environment.

Dedicated in 1967 to provide residential treatment and support services to intellectually disabled children and adolescents, Lynett Village is located on the Morgan Highway Campus in Scranton. In 1980, it was licensed as an Intermediate Care Facility for adults with intellectual/developmental disabilities. Lynett Village consists of three residential units with 14 residents each.

ICF/ID

Adult daycare, senior daycare, and vocational training are provided for all ICF/ID residents according to their specific needs within Allied Services’ system and with other licensed program providers. Throughout such programs, our individuals work with highly qualified professionals in the areas of cognition, communication, socialization, and vocational skills. A wide variety of recreational activities is also provided which is designed to meet each individual’s leisure needs and interests. All individuals receive the benefit of continuous active treatment, supervision, medical care, and ongoing psychological and social services.

Our community-based ICFs opened in the mid to late 1980s and continue to provide active treatment services in the Scranton and Carbondale areas:

CM 1 (ICF/ID) Located in the Green Ridge section of Scranton, this program is home to six individuals.
CM 2 (ICF/ID) Located in Carbondale, this program is home to six individuals.
CM 3 (ICF/ID) Located in Carbondale, this program is home to seven individuals.
CM 4 Located in Taylor, three individuals reside in this home with 24-hour daily supervision.

Community Living Arrangement Waiver Program

Allied Services’ CLA Waiver Program opened in 2001 and is home to three individuals. Around the clock supervision is provided by Certified Nurse Aides in a quiet residential neighborhood.

For Consideration for admission to an ICF, an individual must:

Be at least 18 years of age
Have an IQ of 70 or below
Have significant limitations in two or more adaptive skill areas
Have documented developmental disability from childhood

Once the criteria for admission are met and the Admissions Committee members have reviewed the materials submitted by the Base Service Unit Supports Coordinator, a pre-admission visit is arranged by the Assistant Director of Developmental Services and an interview is conducted and a tour is given.

Upon completion of a pre-placement interview, the Admissions Committee will make a final determination and the referring agency will be informed of a decision within two weeks. All referrals for service are considered without regard to race, color, religious creed, disability, handicap, ancestry, national origin or age. If accepted, the individual is placed on the waiting list.

When a vacancy occurs, the team process is implemented and coordinated by the Assistant Director in choosing an individual from the waiting list who can best benefit from placement at that time.

Required information for admission includes:

A physical examination completed within the past three months that contains a statement from a physician indicating the individual is free from communicable diseases.
A detailed evaluation of pertinent medical conditions.
A chest x-ray or PPD done within the past three months.
Basic laboratory evaluations including CBC, Serology and Hepatitis-B Surface Antigen within one month.
A dental evaluation within one year.
A Psychological Evaluation and Adaptive Behavior Assessment completed within the past three months.
A Social History completed within the past three months.
Physical and Occupational therapy evaluations, if applicable.
Original Social Security, Medical Assistance, Medicare, or any other medical insurance cards.
