Access quality care from home

Are you or a loved one struggling with independence at home? Are you having problems getting around or managing a chronic condition? Have you avoided seeking medical treatment or follow-up care, due to concerns relating to the pandemic?

If you’re delaying care for a new or pre-existing health condition, you’re not alone. According to the Centers for Disease Prevention and Control, an estimated 40.9% of U.S. adults have avoided medical care because of concerns about COVID-19, and people with multiple underlying medical conditions, such as diabetes or hypertension, are significantly more likely to avoid urgent or emergency care compared to people without these conditions.

Avoiding or delaying medical care can lead to a host of adverse health outcomes, especially if you are living with a chronic condition. We understand that you may feel anxious about leaving home to receive necessary healthcare services. That’s why we’re finding new ways to protect our patients and help them to safely access the care they need to live and feel well.

We can help if you are:

  • De-conditioned due to pneumonia, flu, or another illness
  • Recovered from COVID-19 and struggling with mobility, weakness, and other symptoms
  • Experiencing weakness, fatigue, or reduced endurance
  • Living with pain due to illness, injury, or a chronic condition
  • Struggling with mobility or having difficulty with everyday activities
  • Living with Multiple Sclerosis, Parkinson’s disease, or other neurological condition
  • Recently recovered from or discharged from hospital following a stroke
  • Experiencing balance deficits, vertigo, or gait dysfunction
  • Dealing with discomfort from neuropathy, arthritis, or back pain
