Allied Services’ 371-bed Skilled Nursing & Rehab Center provides comprehensive care for both short and long term patients. For many patients, it serves as a transition program, rehabilitating them for their return home with home health or outpatient services, or to an inpatient stay at Allied Rehab Hospital or Heinz Rehab Hospital, if appropriate.
Long Term Care
Skilled nursing care is designed for people who can no longer live safely and independently at home or at an assisted living facility. Typically, people in need of this type of service require long-term, 24-hour care by a team of skilled clinicians including nurses, physical therapists, and occupational therapists. The most common needs of residents at skilled nursing centers include daily medical management, and assistance with activities of daily living. At Allied Services Skilled Nursing & Rehabilitation Center we offer specialized respiratory services, wound care, pain management, behavior management, and care for people with Alzheimer’s disease and other related dementias.
For patients requiring an extended placement, our long term care program offers specialized nursing care, diverse recreational activities and supportive social services catered to the patients individual needs. The physician oversees the medical care along with the interdisciplinary team of licensed professionals. Through the planning process, we are able to follow a patient-centered approach that coordinates the patient’s goals and lifestyle plan with our treatment.
Our transitional rehab track provides patients with intensive, 24-hour nursing and/or therapy services, or as a bridge between an acute care hospital stay and their return home. Often times, patients cannot tolerate or do not require the intensity of therapy delivered in an inpatient rehab hospital. Alternatively, a patient’s insurance benefit may not allow inpatient rehabilitation for their condition. Under the direction of a physiatrist, transitional rehab patients individualized occupational therapy, physical therapy, and/or speech therapy in amounts that fit their endurance level and medical condition. This can range from 30 minutes to two and a half hours per day. Typically, patients in Transitional Rehabilitation are discharged in about 20 to 40 days. Patients may be discharged home with outpatient rehab or home health services, or transitioned to inpatient rehabilitation with the goal of returning home.

Serve as a resident advocate.

Coordinate each resident’s discharge, using community agencies and resources
The Dietary Services Department at the Allied Services Skilled Nursing & Rehabilitation Center prides itself on providing nutritionally balanced meals that appeal to the diverse needs of our residents. Upon admission, every resident receives a nutritional assessment by the Dietitian to establish dietary needs and meal preferences. To help ensure resident satisfaction, the Department has a Food Committee that residents may serve on to exchange menu item ideas and to help keep the menus new and fresh.
Once a year the Dietary Department has a food show. Vendors come to the show to let residents and staff sample new food selections. The Registered Dietitian gets feedback from residents about which of the new foods are their favorites. There are also smaller events throughout the year, like coffee tastings, so residents can continue to give feedback to the staff about what food and beverages they like best.
