Five Star Rating for Transitional Care at Heinz Rehab Hospital

Transitional Care at Heinz Rehab Hospital in Wilkes-Barre recently earned a five star rating from Nursing Home Compare, Medicare’s online comparison tool. The five star rating earned by Transitional Care at Heinz Rehab Hospital reflects superior performance in health inspections, quality of care and staffing.

Nursing Home Compare allows individuals to find and compare nursing homes certified by Medicare and Medicaid. Data used to determined facility ratings comes from 3 key sources: the Centers for Medicare & Medicaid Services’ (CMS) health inspection database, a national database of resident clinical data known as the Minimum Data Set (MDS), and Medicare claims data.

In 2018, Transitional Care at Heinz Rehab Hospital expanded to forty-five beds in response to growing demand for this level of care. The short-stay rehabilitation unit provides round-the-clock nursing care coupled with access to the latest rehab therapies and technologies.

In the report released by Nursing Home Compare in August, 2020, only two facilities in Luzerne County earned a five star rating: Transitional Care at Heinz Rehab Hospital and Allied Services Center City Skilled Nursing.
