Transitional rehabilitation is used when patients require intensive, 24-hour nursing and/or therapy services, but cannot tolerate or do not require the intensity of therapy delivered in an inpatient rehab hospital. Alternatively, transitional rehab is used when a patient?s insurance benefit does not allow inpatient rehabilitation for their condition. Transitional Rehabilitation patients receive therapy in amounts that fit their endurance level and medical condition. Usually this means between 30 minutes and 2 hours of therapy, five days a week. Typically, patients in Transitional Rehabilitation are discharged in about 20-40 days. Patients may be discharged home with outpatient rehab or home health services, or transitioned to inpatient rehabilitation with the goal of returning home.
Heinz Senior Rehab Care Unit
Transitional rehab is provided at The Heinz Senior Rehab Care Unit in Wilkes-Barre, and at our Skilled Nursing Center in Scranton. The Heinz Senior Rehab Care Unit is designed for senior adults 50 and above who need a helping hand to fully recover from illness, injury, or surgery. Unlike many nursing homes, it is not a destination. It is a place of transition. Our goal is for residents to return to their own homes when they are well.
The Heinz Senior Rehab Care Unit has 21 beds, one of which is a private, negative pressure room. Negative room pressure is an isolation technique used to prevent cross-contaminations from room to room. It includes a ventilation system that generates negative pressure to allow air to flow into the isolation room but not escape from the room, as air will naturally flow from areas with higher pressure to areas with lower pressure, thereby preventing contaminated air from escaping the room. This technique to used for isolation of patients with airborne contagious diseases such as tuberculosis, measles, or chickenpox.

Standout Transitional Rehab*
Allied Services Heinz Senior Rehab Unit was ranked as a 4-Star facility on the Centers for Medicare and Medicaid Services (CMS) Nursing Home Compare website. Licensed as a nursing home, our transitional rehab unit was one of only two facilities in Luzerne County PA to have a 4-Star Rating. The rating system measures a facility?s performance on state health inspection results, nurse staffing levels, and performance on select quality measures. Overall ratings range from 1 to 5 stars, 5 stars being the best. In addition to the overall rating, ratings are also provided for each contributing measures. The Heinz Senior Rehab Unit was the only Luzerne County facility to have a 5-Star rating for nurse staffing levels. There is considerable evidence of a relationship between nursing home staffing levels, staffing stability, and resident outcomes. The CMS Staffing Study found a clear association between nurse staffing ratios and nursing home quality of care. Our Skilled Nursing & Rehab Center in Scranton achieved a 4-Star rating on the nurse staffing measure.
