Patients typically require post-acute care to help them recover from an illness, injury or surgery, or to manage the symptoms of a chronic condition. Because each patient’s needs are different, different types of care, or different combinations of care types are required. Allied Services Integrated Health System offers a full continuum of post-acute care services.
Inpatient RehabilitationInpatient rehab is required when patients require intensive (24 hour) therapy services and medical management. Inpatient rehabilitation is delivered in a specialized hospital that focuses on physical, occupational, and/or speech therapy. Care in this setting is coordinated by a doctor who specializes in rehabilitation medicine. Patients also receive care from nurses with specialized rehabilitation training who reinforce therapy goals in addition to managing the patient?s medical needs. The inpatient rehab length of stay is about 10-14 days. After inpatient rehab, most patients return home often with outpatient rehab or home health services. |
Outpatient RehabilitationOutpatient rehab is used for people who are living safely at home, but require rehab therapy to help them live free of physical limitation or pain. Outpatient rehab is delivered at specialized centers, so people receiving this level of care are able to travel. In most instances people require physical therapy; however, others may require occupational therapy, speech therapy, or some combination of these different types of therapy. Usually patients receive about 12-15 one-hour therapy sessions over the course of a four-week period. After Outpatient Rehab most people are discharged with no further services. |
Skilled NursingSkilled nursing care is used for people who can no longer live safely and independently at home or at an assisted living facility. Typically, people in need of this type of service require long-term, 24 hour care by a team of skilled clinicians including nurses, physical therapists, and occupational therapists. The most common needs of residents at skilled nursing centers include daily medical management, and assistance with activities of daily living. Our Skilled Nursing & Rehab Center offers specialized respiratory services, wound care, pain management, behavior management, and care for people with Alzheimer?s Disease and other related dementias. |
Transitional RehabTransitional rehab is used when patients require intensive (24 hour) nursing and/or therapy services, but cannot tolerate or do not require the intensity of therapy delivered in an inpatient rehab hospital. Alternatively, transitional rehab is used when a patient?s insurance benefit does not allow inpatient rehabilitation for their condition. Transitional rehab is provided at a skilled nursing & rehab center. Transitional Rehabilitation patients receive therapy in amounts that fit their endurance level and medical condition. Usually this means between 30 minutes and 2 hours of therapy, five days a week. Typically, patients in Transitional Rehabilitation are discharged in about 20-40 days. Patients may be discharged home with outpatient rehab or home health services, or transitioned to inpatient rehabilitation with the goal of returning home. |
Community ServicesWhen Allied Services was founded over 50 years ago, the first program was Vocational, so that persons with disabilities could be trained for real jobs in the community. In addition to its many rehabilitation medicine programs that have evolved since then, Allied continues the tradition of offering programs that support its clients and consumers in gaining life skills such as work skills, life skills, and academic skills so that they can be independent and successful in spite of mental or physical challenges. |
Home HealthHome health services are used for people who can live safely at home, but require the services of a skilled clinician. Patients who require home health services are medically home bound and generally only leave their home for physician appointments. Typically, patients receive a combination of nursing, therapy, medical social work and or home health aide visits over a 60-day period. After Home Health services many people are discharged with no further services. Others may transition to outpatient rehab, or continue Home Health services for longer periods of time to manage a chronic health condition. Our 24 hour on-call service area includes Lackawanna, Luzerne, Wayne, and Wyoming counties. Services are also available in Monroe, Pike, and Susquehanna counties on a case-by-case basis. |
In-Home CareIn Home Services? trained compassionate attendants assist with personal care needs (everyday activities like dressing and grooming), homemaker services (meal preparation, shopping, laundry, and light housekeeping tasks), respite care, determining the type and frequency a person need services, fiscal management services, and personal emergency response installation and maintenance. These services allow the elderly or disabled to live independently in their home. Services are available in 22 counties including: Berks, Bradford, Carbon, Centre, Clinton, Columbia, Lackawanna, Lehigh, Luzerne, Lycoming, Monroe, Montour, Northhampton, Northumberland, Pike, Schuylkill, Snyder, Sullivan, Susquehanna, Tioga, Union, Wayne, and Wyoming. |
Personal CarePersonal Care facilities offer a group living arrangement that provides help with activities of daily living, and getting to appointments. Patients do not require 24 hour supervision by a skilled clinician. Residents of Personal Care facilities often live in their own room or apartment. Social and recreational activities are usually provided. Some Personal Care facilities have health services on site. Personal Care services are typically not covered by medical insurance. Residents usually pay a monthly rent out-of-pocket. Residents of Personal Care facilities may require short-term post acute services such as inpatient rehab, Rehab rehab or home health while they are residents at their Personal Care Facility. |
